| In person: There are two locations: The Ft Lauderdale office is located at 2421-A SW 6th Avenue. The hours of operation are from 8:00 am to 4:30 pm, Monday thru Friday, except the third Friday of each month when the office opens at 9:00 am. The Hollywood office is located at 7999 Pembroke Road. The hours of operation are from 8:00 am to 4:30 pm, Monday thru Friday. Closed for lunch 12 noon to 1:00 pm. Also, closed every third Friday of the month. | ||||||||||||||||

Application for Birth Certificate Birth Certificates ordered through our regular mail order service take 10 to 14 business days to process and must include a self-addressed, stamped envelope. For an additional fee we also offer Expedite ($10) and Overnight ($21) Processing services.

| Personal Checks require the person’s name, address, and phone number be imprinted on the check. This information must match the Driver's License or State Identification or other government issued identification that includes a photo and current address. |

IDENTIFICATION: Birth Certificates are not public record and may be issued to only certain individuals. Applicants for these certificates must present an official, picture identification card, such as their Driver’s License, Pass Port, or other State Identification card. All mail or facsimile orders must include a copy of the applicant’s Drivers License or State Identification Card.

RESTRICTIONS: Birth Certificates can be issued only to the registrant (the child named on the record) if of legal age, parent(s) (name(s) that are stated on the birth certificate), guardian or other legal representative or by court order. In the case of a deceased registrant, upon receipt of the Death certificate of the decedent, birth certificate can be issued to the spouse, child, grandchild, sibling, if of legal age, or to the legal representative of any such persons, as well as to the parent.
The Broward County Vital Statistics office DOES NOT handle marriage licenses, divorce decrees, probate, wills, or Social Security.
