The Outreach Planning Group (OPG) serves as a venue for Broward County Health Department staff and community partners to share information about community health fairs. The purpose of the OPG is to facilitate the coordination of Broward County Health Department staff's participation in events, provide an opportunity for networking and a mechanism to obtain feedback for improving subsequent events.

Instructions for Requesting Vendors Through the OPG:

Need Vendors for your health event?

• You will need to complete and return the "Request for Health Vendors" form by email. Send the completed form to: chd06post@doh.state.fl.us. This is a form that the Broward County Health Department OPG Coordinator uses to gather information on community events and share with members of the OPG. The form will be sent in an email to an OPG distribution list.
   
• You will then be contacted by vendors available to participate in the event. It is recommended that the event planning committee continue to contact outside agencies for support and participation and is encouraged to follow up after the form has been submitted.
